Health and consent form
Please complete, sign and return the health and consent form before your doggie is picked-up for the first enclosed dog park visit. We do not need your dog's health or vaccination records, only your signature indicating all vaccines are current and will be maintained during DPE's dog daycare service time.
We will administer support if there is an accident. We will also promptly contact you with the status, and transport your dog to your vet listed on your health form, at your request. We will also stay with your dog until you or a family member arrives.
We love all doggies but our insurance company doesn't
Our insurance policy prevents DPE from transporting and providing service to Pit Bulls/ American Stafford Terriers, Rottweilers and Chow Chows.
Aggression policy
Accidents happen. Not all doggies play well together. We do our best to introduce and onboard each dog. We plan a "meet and greet" to identify behaviors and temperament issues that could be a problem at an enclosed dog parks. If aggression issues cannot be solved, we reserve to right to terminate service.
Life happens... we get it. A 24 cancellation courtesy notice would be helpful.
Because we take special care and time to group dogs that play well together we greatly appreciate a 24 hour cancellation notice to regroup or fill spots. We understand last minute things come up. We will work with you on changes and rescheduling of appointments.
